- If you are a new eTherapy client, please complete all the following steps:
- Complete a Release Form. This form acknowledges our terms and conditions.
- Complete an Intake Form. This form allows you to share information on your situation that will help in assessing your needs.
- Proper completion of these forms will send Mary Ann an email and attach the release form and intake form requesting a counseling session.
- Finally, you will be taken to the Payment Center hosted by PayPal to complete your transaction. Payment for each 55 minute session is $65. Remember to ‘unclick’ shipping on the Paypal page or Paypal will charge you an extra $13.
- Mary Ann will respond to you when she receives your email. You will then discuss available session opportunities.
